Young people who are leaving care and moving into the
adult world need more than a wave goodbye. Pathway
planning is a long and measured process, involving other
agencies and, most importantly, the young person
themselves. It does not always go smoothly but social
workers have a responsibility to ensure that plans and
safeguards are in place that remain available in the months
and years to come.This course considers a structured
approach to pathway planning, highlighting the importance
of contingency planning and provide an opportunity for
participants to try out the permanency planning format.
Learning outcomes cover:
- Skills for workers in working alongside young people
to build security, confidence and relationships in their
long-term placements.
- Research and practice guidelines around a variety of
models of permanence, including links into leaving care.
- How to set measurable objectives.
- Understanding a care plan format.
